Financial Aid Electronic Forms

First Time Users

The first time you access Dynamic Forms, you will be required to create an account by entering a username, password, and other information. After completing the form, you will be given the option to electronically sign and submit the form. You will also be able to save or print a copy for your records.

Electronic Signatures

If you choose not to electronically sign the form, you must print and send the form to the Financial Aid Office for processing. If your form requires parental information or signatures, you will be asked to supply a parent email address before you can access the form. Your parent will be sent a link to complete the parent section of the form. The parent will need to create a user ID and password.

You may submit requested documents electronically through Dynamic Forms. This is a secure site that DSCC uses to provide you with a convenient method to submit the required information. To complete forms electronically:

  • Login to your MyDSCC account
  • Click on the Student Tab
  • Find the Financial Aid Requirements box
  • Select the Aid Year
  • Click on the highlighted requirement link (this will open a new page with Dynamic Forms)
  • Click on Complete This Form
  • Make sure all blanks are completed
  • Electronically sign the form and submit
