DSCC Transcript Request Form

Current students and alumni may request up to six transcripts per semester free-of-charge by completing the DSCC Transcript Request form.

Photo Identification Needed

If you request to pick up your unofficial transcript in person from the Dyersburg Campus One Stop, a state issued or DSCC photo ID will be required. We are sorry, but transcripts cannot be picked up from any of our off-campus locations.
